Physicals and Immunizations

All New Students and students entering Pre-K, Kindergarten, Third Grade, Sixth Grade, Seventh Grade and Eighth Grade must show proof of an updated physical from within the last year.

In addition, all New Students and students entering Pre-K, Kindergarten, Third Grade, Sixth Grade, Seventh Grade and Eighth Grade must have updated Immunization records on file at the school.

Immunizations* Required for School:
By Kindergarten:           

  • 3 doses of Hep B
  • 5 doses of DTaP/DTP
  • 4 doses of Polio
  • 2 doses of MMR
  • 1 dose of Varicella


By 7th Grade:

  • 3 doses of Hep B
  • 1 booster dose of Td
  • 3 doses of Polio
  • 2 doses of MMR
  • 1 dose of Varicella


*Note about immunizations:  the following Massachusetts statute applies to all children in grades pre-kindergarten through twelve, and it provides for exemption from the immunization requirement for medical or religious reasons: “No Child shall, except as hereinafter provided, be admitted to school except upon presentation of a physical certificate that the child has been successfully immunized against diphtheria, pertussis, tetanus, measles and poliomyelitis and such other communicable diseases as may be specified from time to time by the Department of Public Health."
